If you require any vaccinations relating to foreign travel you need to complete a travel questionnaire as below. This will include which countries and areas within countries that you are visiting to determine what vaccinations are required.

Please download and fill in the Travel Risk Assessment Form as early as possible - minimum 6 weeks before travelling
The questionnaire will be forwarded to the nursing team, who will assess your requirements and contact you by phone and arrange any vaccinations if needed
Some vaccinations will require you to be referred to a travel clinic as either these are not available via the NHS or are too complex. These will incur a charge which will be discussed through the travel clinic
Alternatively you can go direct to a travel clinic, a local clinic is based at Woodhouse Medical Practice. Please note a consultation fee of £20.00 will be charged if no vaccinations are purchased from them.
