National Diabetes Prevention Programme

We are supporting you in trying to prevent the onset of Diabetes.

We have started writing to and sending text messages to patients who are eligible to attend the “National Diabetes Prevention Programme” (NDPP).¬† The NDPP¬† is for people whose blood tests suggest they are at risk of developing diabetes.

If you have received notification that you are eligible for the NDPP then either reply to the text message we sent you, phone our receptionists (0113 2300 391) or drop an email to alwoodleymedicalcentre@nhs.net with your name, address and date of birth and we will arrange the referral.
